Regional Resource Mapping Community Assessment Surveys | State of Illinois Office of the Illinois Courts

The Illinois Supreme Court Task Force on Improving the Court and Community Response to Mental Health and Co-Occurring Disorders (Task Force) is conducting a Community Assessment to supplement the Regional Resource Mapping Workshops. The Community Assessment is modeled after the SIMPLE Scorecard from Wayne State University's Center for Behavioral Health and Justice.

As court and community stakeholders, the Task Force is requesting you complete this Community Assessment which will inform future strategic and actions plans.

The Community Assessment is broken into three sections to correspond with the Regional Resource Mapping Sessions although all three surveys are open for response through May 6th, 2022.

The survey tool allows for respondents to start the survey and return for completion at a later date if time is needed to gather information ensuring a thorough response.

The first of three surveys in the Community Assessment process is the longest and is expected to take approximately 30 minutes to complete once all of the relevant information is gathered. The second and third surveys are expected to take approximately 15 minutes each.
