Illinois Supreme Court Commission on Access to Justice’s Forms Committee welcomes two new members

The Forms Committee was established in 2012 as a cornerstone project of the Access to Justice Commission to govern the development of statewide standardized court forms.

The Committee is chaired by the Honorable Michael Fiello (1st Judicial Circuit) and the Honorable Catherine Schneider (Circuit Court of Cook County) and consists of ten volunteer members representing a broad range of court stakeholder groups across Illinois. The Committee oversees the work of 13 drafting subcommittees responsible for creating forms in various areas of law.

Pursuant to Supreme Court Rule 10-101 and M.R. 25401, the initiative focuses on developing forms in areas of law with a high volume of self-represented litigants or where standardized court forms enhance access to justice or court efficiency. Statewide forms serve as a vital access point for self-represented litigants navigating the court system. Currently, 70 form sets and approximately 353 individual statewide standardized forms are published on the Illinois Courts website.

We are pleased to announce that Hallē Eichert, Director of the Kane County Law Library & Self Help Legal Center and Timothy Herman, Member, Clark Hill PLC have joined the Forms Committee effective January 1, 2026, and will serve three-year terms. They replace Marilyn Stromborg and Richard Toboz, whose terms concluded on December 31, 2025.
